Identifying Key Characteristics of a Successful CONTINUUM of Care Program (COC)
Long-range Continuum of Care: recognizes that to better serve homeless people and to create affordable permanent housing and supportive housing options, considerable time, energy, and financial resources, as well as linkages to mainstream services and affordable housing are needed.
Comprehensive and Collaborative Continuum of Care: involve the breadth of housing and service options. The plan is designed to meet the unique needs of subpopulations while also developing capacity to serve people who are homeless with multiple needs
Developing a strategic plan for the successful delilvery of a Continuum of Care Program: will incorporate realizable strategies to move homeless individuals and families beyond temporary shelters to permanent housing and self-sufficiency. In addition, we understand that solutions to complex problems require careful development of action plans to achieve the desired objectives on a case by case basis. In order to implement all of the above components our case managers and program directors will work to implement and further develop a continuum of care assessment based on community needs and priorities.
